Informed consent
Physiotherapy is a safe and effective form of treatment for physical concerns. However, all interventions carry the risk of possible side effects and negative responses to treatment which are unique to the individual. Your physiotherapist will inform you of both the benefits and risks of each intervention prior to the commencement of treatment to ensure you achieve your goals whilst being comfortable. You may refuse any treatment or choose to withdraw your consent at any time whilst receiving physiotherapy care.
Dry needling and cupping are valuable adjunct treatments for chronic pain, stiffness, and to deactivate myofascial trigger points. Like any medical procedure, there are possible complications. While these complications are uncommon, they do sometimes occur and must be considered prior to giving consent to the procedure. With the dry needling technique, a fine, flexible and sterile needle is used. The purpose of the needling is to release shortened bands of muscle caused by abnormal functioning of the nervous system. No drugs are injected. Dry needling may cause an increase in pain for one to three days followed by an expected improvement in the overall pain state. The increased pain is related to overactive shortened muscle bands that have not been released and to the soreness caused by the “twitching” of the muscles. Any time a needle is used there is a risk of infection. However, we are using new, disposable and sterile needles, and infections are extremely rare. A needle may be placed inadvertently in an artery or vein. If an artery or vein is punctured with the needle, a hematoma (or bruise) will develop. If a nerve is touched, it may cause paraesthesia (a prickling sensation) which is usually brief, but it may continue for a couple of days. When a needle is placed close to the chest wall, there is a rare possibility of a pneumothorax (air in the chest cavity). Fortunately, all these complications are not fatal and are readily reversible. An ancillary technique that we also use is cupping where suction cups are applied to the skin with the therapeutic aim of increasing nutrient-rich blood supply to the injured area, decreasing congestion as well as improving general range of motion. A temporary side effect of cupping is discoloration of the area cupped. These marks are temporary and will fade within a few days. Myofascial dry needling and cupping are generally safe for most people however patients are requested to inform practitioners about conditions such as pregnancy, pacemakers, a history of vascular diseases such as DVT or varicose veins, open wounds, and the use of blood thinners or immunosuppressant medications prior to the treatment. You may refuse these treatments at any time.
For treatment of a person under the age of 18, this form must be read and agreed upon by a legal custodian, and the presence of a caregiver is requested at the time of service.
If an individual is unable to give oral, written, or implied consent at any time, this form must be signed by a legal representative, and the presence of this legal representative is requested at the time of service.
States of undress
At times, your physiotherapist may ask you to reveal a body part for examination or treatment which requires various states of undress. Whilst this provides your physiotherapist with valuable information regarding your injury or concern, it is not essential. You are encouraged to voice any concerns regarding undressing prior to your appointment so that your physiotherapist can plan the consultation ahead of time.
Women’s and Men’s Health
Please note that physiotherapists at Globe Athletic Physiotherapy are not currently qualified to diagnose pelvic floor related conditions, including performing an internal examination, but can assist you in the treatment of these conditions. This may include education, management strategies, manual therapy at regions seperate to the pelvic floor itself and exercise prescription and instruction. As such, we ask that you receive an initial consultation with a women’s or men’s health physiotherapist and then return to Globe Athletic with a diagnosis so that we can begin treatment. On occasion, your physiotherapist may also recommend you continue treatment with a specialist when they feel it is outside their scope of practice. We are also able to provide these referrals for you if you are unsure where to go.

or marketing campaigns. If any part of the finalised media is identifiable, your physiotherapist will obtain further consent from you prior to sharing on a public platform. You can choose to withdraw this consent at any time and the media will be removed promptly.
Individual response to treatment
Everyone will respond differently to physiotherapeutic interventions and some people may take shorter or longer periods of time to recover from a similar injury. This is due to factors that impact healing such as co-morbidities, lifestyle, mental health and the amount of intervention received. If you are concerned about your response to treatment, please discuss this with your physiotherapist so we can help you achieve your goals in a timely fashion
Pilates classes
An initial physiotherapy consultation is required for all clients wanting to join clinical classes. Bookings for each fitness or clinical class will close 3 hours prior to the start time and any cancellations within this timeframe or no-shows will be subject to our below cancellation policy. You will also be notified by text with a minimum of 3 hours’ notice if a class is to be canceled due to low attendance.
Fees
Private paying patients are required to cover their fees at the time of service. If you cannot pay the entire invoice on the day, you will have 3 business days to pay any remaining balance or you may be subject to additional fees and charges associated with debt collection. No further services will be available for booking until the previous service is paid in full. Private health insurers usually cover some, but not all associated fees and you are likely required to pay a gap payment. We have Medipass integrated with our practice management software to make healthcare claims easier for you. If your insurer is not registered with Medipass, you will need to pay in full before claiming reimbursement from the insurer online. If you are on a Chronic Disease Management/ Enhanced Patient Care plan, a $25 gap payment will be required on the day and the remainder of the account will be sent directly to Medicare as required. Eligible concession card holders will also receive a 10% discount on services at Globe Athletic Physiotherapy. These can be applied in person after your appointment. If an interpreter is required to facilitate communication between yourself and the physiotherapist this will be incurred at your own cost. At times, products will be recommended to you to assist in your treatment plan but purchase of these products is optional and up to your discretion.
Cancellations
Your physiotherapist will recommend a treatment plan that fits in with your lifestyle and treatment goals. However, we know that life sometimes gets in the way and you will need to reschedule your appointments. When this occurs, please make contact us 24 hours prior to your appointment. If you do not provide 24 hours’ notice, you will incur a cancellation fee of 50% of the original service or class price unless extenuating circumstances apply, such as family emergencies. If you need to cancel due to acute illness the cancellation fee will be waived if a subsequent consultation is rebooked within 7 days or the original date and time, pending practitioner availability. If you do not attend your appointment without contacting the clinic first you are required to pay to entire original service price fee. This also applies to class packs, where a full class will be deducted from your pack if you do not attend or cancel within 3hours of the scheduled class. Please be considerate of your physiotherapists time and refrain from texting or calling others during your appointment unless it directly affects your healthcare.
Feedback
Word-of-mouth referrals are a great compliment to our clinic and we encourage you to refer any family and friends you feel would benefit from our service or write a Google review online. Conversely, any suggestions, comments, or complaints about the service you have received can be submitted to physio@globeathletic.com.au and will be addressed appropriately. Please note, that no refunds are issued on serviced provided but are on products sold if faulty and proof of purchase is received. No refunds are issued for change of mind.
Privacy policy
- Globe Athletic Physiotherapy adheres to the Australian Privacy Principles contained in the Privacy Act 1988 (Cth). This policy sets out how we collect and treat your personal information.
- “Personal information” is information we hold which is identifiable as being about you.
- 2a. Globe Athletic physiotherapy will, from time to time, receive and store personal information you enter onto our website, provided to us directly or given to us in other forms.
- 2b. You may provide basic information such as your name, phone number, address and email address to enable us to send information, provide updates and process your product or service order. We may collect additional information at other times, including but not limited to, when you provide feedback, when you provide information about your personal or business affairs, change your content or email preference, respond to surveys and/or promotions, provide financial or credit card information, or communicate with our customer support.
- 2c. Additionally, we may also collect any other information you provide while interacting with us.
How we collect your personal information
Globe Athletic physiotherapy collects personal information from you in a variety of ways, including when you interact with us electronically or in person, when you access our website and when we provide our services to you. We may receive personal information from third parties. If we do, we will protect it as set out in this Privacy Policy.
Use of your personal information
- You consent to your name and email is provided for the purpose of being provided membership information from time to time. You can unsubscribe from this mailing list when contacted by Globe Athletic
- Globe Athletic physiotherapy may use personal information collected from you to provide you with information, updates and our services. We may also make you aware of new and additional products, services and opportunities available to you. We may use your personal information to improve our products and services and better understand your needs.
- Globe Athletic physiotherapy may contact you by a variety of measures including, but not limited to telephone, email, sms or mail.
Disclosure of your personal information
- We may disclose your personal information to any of our employees, officers, insurers, professional advisers, agents, suppliers or subcontractors insofar as reasonably necessary for the purposes set out in this Policy. Personal information is only supplied to a third party when it is required for the delivery of our services.
- We may from time to time need to disclose personal information to comply with a legal requirement, such as a law, regulation, court order, subpoena, warrant, in the course of a legal proceeding or in response to a law enforcement agency request.
- We may also use your personal information to protect the copyright, trademarks, legal rights, property or safety of Globe Athletic physiotherapy www.globeathletic.com.au its customers or third parties.
- Information that we collect may from time to time be stored, processed in or transferred between parties located in countries outside of Australia.
- If there is a change of control in our business or a sale or transfer of business assets, we reserve the right to transfer to the extent permissible at law our user databases, together with any personal information and non-personal information contained in those databases. This information may be disclosed to a potential purchaser under an agreement to maintain confidentiality. We would seek to only disclose information in good faith and where required by any of the above circumstances.
- By providing us with personal information, you consent to the terms of this Privacy Policy and the types of disclosure covered by this Policy. Where we disclose your personal information to third parties, we will request that the third party follow this Policy regarding handling your personal information.
Security of your personal information
- Globe Athletic physiotherapy committed to ensuring that the information you provide to us is secure. In order to prevent unauthorised access or disclosure, we have put in place suitable physical, electronic and managerial procedures to safeguard and secure information and protect it from misuse, interference, loss and unauthorised access, modification and disclosure.
- The transmission and exchange of information is carried out at your own risk. We cannot guarantee the security of any information that you transmit to us, or receive from us. Although we take measures to safeguard against unauthorised disclosures of information, we cannot assure you that personal information that we collect will not be disclosed in a manner that is inconsistent with this Privacy Policy.
Access to your personal information
- You may request details of personal information that we hold about you in accordance with the provisions of the Privacy Act 1988 (Cth). A small administrative fee may be payable for the provision of information. If you would like a copy of the information which we hold about you or believe that any information we hold on you is inaccurate, out of date, incomplete, irrelevant or misleading, please email us at hello@globeathletic.com.au
- We reserve the right to refuse to provide you with information that we hold about you, in certain circumstances set out in the Privacy Act.
Complaints about privacy

- Please be aware that we may change this Privacy Policy in the future.
